(1) The survey is anonymous and nobody will ever see your response. The information gathered is only intended for management to get an unbiased and honest view of harassment in the workplace in the form of a data map without any names or numbers.

(2) The survey consists of only 18 multiple choice questions and will take less than a minute to complete. Please do not "REFRESH" the page during the survey.

(3) Your employer is responsible to prevent harassment in the workplace and on the premises, including suppliers, customers or external contractors. They are therefor included in the context of the questions.
